An entire human cranium can now be added to the growing list of 3D-printed body parts that includes a fingertip, a hand, prosthetic eyes, arms, a jaw, and even a new foot for a duck. The plastic skull was made by an Australian firm and placed on the brain of a Dutch woman at Utrecht University's University Medical Center in the Netherlands. The operation, which lasted 23 hours, took place about three months ago, and Dutch News just reported that the patient has returned to work -- plastic noggin and all. "The patient has her sight back entirely, is symptom-free, is back to work, and it is almost impossible to see that she's ever had surgery," lead neurologist Ben Verweij said in a statement. Prior to the procedure, the woman's skull was more than three times thicker than a normal skull due to a rare condition. The increased thickness caused the woman's skull to press on her brain, leading to severe headaches and vision loss. Although the report doesn't name the condition, Camurati-Engelmann disease is among the ailments that can cause skull bones to thicken. "Implants used to be made by hand in the operating theater using a sort of cement which was far from ideal," Verweij said of the procedure, according to Dutch News. "Using 3D printing we can make one to the exact size. This not only has great cosmetic advantages, but patients' brain function often recovers better than using the old method." Verweiji says that although portions of skulls have been swapped out in the past, this is the first time an entire cranium has been replaced in a patient. Compartment Syndrome

This is a syndrome that results from swelling of a muscle. The muscle is retained in its fixed volume fascia, however, and thus the swelling of the muscle causes occlusion of the blood supply. this can result in infarction, and can cause an ischaemic contraction known as Volkmann’s Ischaemic Contracture.   It typically occurs in the forearm and calf, but can also occur in the thigh and foot.

Henoch Schonlein Purpura (HSP)

This is a vasculitis that most commonly occurs in children. It tends to only affect the small vessels, and typically presents with: Palpable purpura – red/purple discolorations in the skin, often on the extensor surfaces of the feet, legs, arms, or sometimes on the buttocks. The rash may initially resemble urtricaria, but later becomes palpable. GI disturbance – may include colicky abdominal pain, abdominal tenderness, melena – occurs in 50% of patients

Talipes Equinovarus (Club Foot)

Talipes equinovarus is a deformity of the foot and ankle commonly found at birth (the foot is fully plantar-flexed and there is midtarsal adduction causing a varus deformity). It is estimated to affect 1 in 1000 births in the UK. Boys are twice as likely to be affected as girls. The cause of the deformity is not completely clear however it is speculated that it is due to mechanical pressure in utero. There also appears to be a genetic link.

Drugs for neuropathic pain

The patient is a 63 year old freelance editor with type 2 diabetes diagnosed about five years ago that is relatively well controlled with insulin. He has early signs of retinopathy, with normal kidney function and electrocardiogram. Lipid values are normal with diet and atorvastatin 20 mg/day. He developed autonomic and peripheral neuropathy a few months ago, and now experiences postural hypotension and burning pain and clumsiness in his feet. His pain makes concentration and falling asleep difficult. He asks his general practitioner for painkillers to help him continue working.

A weak leg

A 50 year old right handed man presented with weakness of his right leg and arm of two months’ duration. The symptoms had come on over several days, during which he described a “dead leg” and he noticed his foot dragging while walking. On examination he had right lower limb pyramidal type distal weakness with a foot drop, power was graded at 4/5, and he had an upgoing plantar reflex. A computed tomogram of the brain showed a left parietal lesion with surrounding oedema but no midline shift, so contrast enhanced magnetic resonance imaging was undertaken (figs 1⇓ and 2⇓). Analysis of the cerebrospinal fluid showed raised protein with normal cell counts, normal angiotensin converting enzyme values, oligoclonal bands, IgG of 185 mg/L (normal value <40), and a negative viral polymerase chain reaction. Serum immunoglobulins and vitamin B12 values were normal; viral antibodies and antibodies for spirochetes were negative. He had no history of a neurological deficit.
